If you lack a suitable comparator and strong supporting literature, while also introducing something genuinely new, a clinical investigation is much more likely. It is better to plan for it early than discover the requirement halfway through the regulatory review.

Novel combinations of functions: Additional evidence may be required when combining established technologies creates new clinical questions.

Ultimately, the need for a study depends on the evidence gap. The weaker the existing evidence for the specific device, claim, or population, the more likely a Medical Device clinical investigation will be required.

Waiting until the technical file is nearly complete to address clinical evidence needs can become expensive. You may end up rushing a study to fill a late-stage evidence gap or receiving a Notified Body deficiency letter or FDA Additional Information request, which can add months to the timeline.
